Waiver for Persons with Physical Disabilities (PD)
This waiver serves recipients of all ages who have a
documented physical disability, meet Nursing Facility Level of Care and would require imminent placement in a nursing facility if HCBS or other supports are not available. Eligibility for the waiver is determined by the Division of Health Care
Financing and Policy (DHCFP) and Aging and Disability Services Division (ADSD). The following services are
offered in this waiver:
- Case Management
- Homemaker Services
- Chore Services
- Respite
- Environmental Accessibility Adaptations
- Specialized Medical Equipment and
Supplies
- Personal Emergency Response System
(PERS)
- Assisted Living Services
- Home Delivered Meals
- Attendant Care Services

Provider
To
enroll as a PD Waiver provider, complete the enrollment process through Nevada Medicaid's fiscal agent.
