Home Health
Complete the Home Health Agency Prior Authorization Request (FA-16A) and submit through the Provider Web Portal. Home Health services provide medically necessary skilled interventions, including:
- Nursing Services
- Physical Therapy
- Occupational Therapy
- Speech Therapy
- Respiratory Therapy
- Dietician Services
All Home Health services require a doctor’s order and prior authorization.

Recipient:
A doctor’s order is required for Home Health services. Your healthcare provider will submit the doctor’s order to a Medicaid enrolled Home Health Agency. The agency will submit the required documentation to Nevada Medicaid's fiscal agent to request prior authorization for Home Health services.
Provider:
