Waiver for Persons with Physical Disabilities(PD)
This waiver serves recipients of all ages who have a
documented physical disability, as determined by the Division of Health Care
Financing and Policy (DHCFP) and the Aging and Disability Services Division (ADSD),
and who maintain the required Level of Care (LOC) (admission into a Nursing
Facility within 30 days if waiver services or other supports were not
available). The following services are
offered in this waiver:
- Environmental Accessibility Adaptations
- Specialized Medical Equipment and
Supplies
- Personal Emergency Response System
(PERS)

To request services contact the appropriate ADSD Office.
Provider
To
enroll as a Waiver for Persons with Physical Disabilities provider complete the enrollment process through Nevada Medicaid's fiscal agent.
